At Coosa Care, we understand that every practice is different. That’s why we offer flexible AWV solutions that fit your workflow and help improve patient satisfaction, reduce costs, and support healthier lives.

Here’s how we make it simple:

  • Medicare roster review – We help identify which of your patients are eligible.

  • Eligibility verification – Confirm who qualifies for G0438 or G0439 visits.

  • Patient outreach – Two options to meet your practice needs:

    • Complete "Care at Home" AWV Support – We collect all required information and tie you (or your qualified PA/NP) into the visit via Telehealth using our remote software, allowing the visit to be fully compliant with CMS guidelines in a single encounter. Your patients never have to leave home.

    • Partial Visit Support – We gather preliminary information for the provider to complete later. This option may require additional scheduling and can sometimes create technical challenges on the patient’s end.
